General Information
For the protection of your child and others, please do not send your child to school with any of the following:

1. Vomiting
2. Runny nose/color discharge
3. Sore throat
4. Fever
5. Rash
6. Pediculus capitis (head lice)
7. Diarrhea
8. Earache and/or eye discharge
9. Persistent cough

Each child has a permanent Health Record. On this card, pertinent information is recorded. Please keep the school notified of any health problems. All information is held confidential. Please feel free to contact the school to discuss any health problems or attendance concerns. NOTE: Children are required to be fever free twenty-four hours before returning to school.

H1N1 Flu Information
District and school administrators are working with U.S. and Illinois education and health agencies to plan for the upcoming flu season to be sure our policies and practices
are appropriate should the H1N1 flu impact our schools. As more information is available, it will be shared with families.

Overall, students and staff are reminded to exercise good personal hygiene habits. The Illinois Department of Public Health suggests the Three C’s:

Clean – Wash your hands frequently to prevent the spread of germs
Cover – Your cough or sneeze with a tissue or sleeve, not your hand
Contain – your germs – stay home if you are sick

Health Forms
In addition to immunizations, the State of Illinois requires the following for all students enrolled in public or private schools:

   Early Childhood and Project Pro - Students need a school physical examination.
   Kindergarten - Students need a school physical, dental, and vision examination.
   2nd Grade - Students need a dental examination.
   6th Grade - Students need a school physical and dental examination.

Additionally, any student enrolling for the first time in Illinois must have an eye exam.

Orland Township Free Immunization Clinics and Health Fairs
Free immunization clinics are held monthly at the Orland Township office located at
14807 S. Ravinia Avenue, Orland Park. All vaccines are administered, free of charge, to Orland Township residents 18 years of age and younger. Back to School Health Fairs are offered twice a year and include free dental screenings, free vision screenings, free immunizations, and $20 school physicals. Please note that clinics and health fairs are for residents of Orland Hills, Orland Park, Tinley Park and unincorporated Orland Township. A written immunization record and proof of residency is required. All Children under the age of 18 must be accompanied by a parent.

Medicine
School Board Policy prohibits any student from carrying any form of medication with them while in school. Extreme cases may warrant the need for medicine to be taken during the day. In this case, parents need to contact the school nurse and complete a release form. The child's physician must also complete another form that describes the medication and dosage. Please click Authorization for Administration of Medication in School to download the necessary forms.

When completed, the medication must be brought to school in the original prescription container and will be dispensed by the nurse. This is not intended to cover the occasional need for medication. Please refer to the District Parent/Student Information Handbook for further clarification regarding the self-administration of asthma medication.

Vision Screening
McAuliffe School conducts a Vision Screening for Kindergarten and 2nd Grade.  The vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor.  Children are not required to undergo the vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an evaluation has been done within the last 12 months.
